2010 Park Ridge Memorial Day Tournament - Application Form & Instructions
(1) Fill out the form below with your team information.
(2) Print two copies of the PRS Memorial Day Tournament Application Form, one to mail, one for your records.
(3) Review the fee structure and remit the proper payment amount(s).
- U8,U9 & U10: $425 per team
- U11, U12, U13 & U14: $475 per team
- U15 & U16: $500 per team
-Early Registration Discount: If registration is received by March 19th, 2010, $25 off per team
(not to be combined with Club Discounts).
-Club Discounts: 5-14 Teams from each Club --- $50 off per team.
15 -24 Teams from one Club --- $60 off per team.
-More than 24 teams contact director for those discounts.
-Registration Deadline is April 16th, 2010 (Final acceptance is subject to a balanced number of teams in each bracket).

(5) Mail the PRS Memorial Day Tournament Application Form and check to the following
