PRS UNIFORM AND FORMS NIGHT

All players and parents must attend Uniform and Forms Night to turn in paperwork and order a uniform.  If you are unable to attend the Uniform and Forms night, please email the respective Travel Director to make arrangements for delivery of the Travel Fee and Forms prior to June 11th for U9 players and June 15th for U10-U14 players.    The Girls Travel Director is Paul Vinopal and the Boys Travel Director is Peter Mierzwa.  All arrangements must be made by June 11th for U9 players and June 15th for U10-U14 players to reserve your child’s spot.

Items to Bring to Meeting

All new and existing PRS Travel players must bring the following items to the meeting:
• Completed IYSA Medical Release and Liability Waiver
• Completed Player Registration Form - IWSL for the Girls and YSSL for the Boys.
• Photocopy of front and back of medical insurance card
• Check for $700 payable to Park Ridge Soccer for Travel Fee
• Completed uniform order form from below:

• Check for uniform order payable to Park Ridge Soccer described below. 

All new PRS Travel players must also bring a photocopy of their birth certificate or Passport for birth date verification and submission to their respective league for issuance of their player pass.

U9 Organizational Meeting

The Uniform and Forms Night for the U9 age group is divided by gender.  The Girls teams will meet from 6:00-7:30 and the Boys teams will meet from 7:30-9:00.  The basic structure of each meeting will consist of the following:
• Introduction to the PRS Travel Program by the Travel Director
• Introduction of the coaches for each team.
• Breakout session by team to select player numbers and obtain digital picture of players.
• Uniform ordering.

Returning PRS Players

We request that the U10-U14 Girls deliver their forms and order any uniform items between 6:00-7:30.  We request that the U10-U14 Boys deliver their forms and order any uniform items between 7:30-9:00.   There is no organizational meeting for your team on this evening unless your coach makes arrangements to meet on this night.  Separate team meetings arranged by your coach may be held at another time.

All fees must be paid in full on or before the Uniform and Forms Night for your age group.  Returning players assigned to a new team may need to order new jerseys if their jersey number already exists on their new team.  Players who remain on an existing team (Orange, White, and Blue) keep their jersey number.

Uniforms

U9 uniform package cost is $121 and includes orange jersey, orange shorts, orange socks, backpack, and 2 practice t-shirts.  In addition to the items listed for U9, new U10-U14 players are required to purchase a blue jersey, blue shorts and blue socks for an additional $59 for a total of $180.  Other optional items are additional practice t-shirts, additional socks, hooded sweatshirt, warm up jacket and pants are available at an additional cost.
