Registration Checklist

BOYS - 6-7:30 PM GIRLS 7:30-9PM

1- <NEW PLAYERS ONLY>Proof of age – copy of birth certificate, passport

2- ALL Players need to fill out the Player Registration form (IWSL for Girls and YSSL for Boys) – found on the PRSoccer.com site under General Info/Forms
3- IYSA Medical Release and Liability Waiver
4- Digital headshot of player for player pass – email to Coach
5- Check made out to “Park Ridge Soccer” in the amount of:
·         U10-U12 - $750
·         U13-U14 - $850
6- Proof of insurance coverage - Copy of insurance card
7- Uniform Orders (New players need all items except as noted)
(Current players only need to replace items as needed)

--Complete Uniform Order form located on the PRSoccer.com form page or at Uniform Order Form-Excel or Uniform Order Form-Web-HTML and bring it to the Uniform Night June 21st.
U10-U14 Players need:
o   1 pair Orange shorts
o   1 pair Orange sox
o   1 Orange jersey
o   2 practice t-shirts
o   PRS backpack (optional)
o   PRS warmup suit (optional)
